Then fill up and submit given online entry form on this page. In given field enter your first name, last name, address-1, address-2, city name, zip code, phone number, email address. Select your State, Birthdate, your phone type. After type word in box which is given. Lastly, you clicks on submit option and automatically receive the one (1) entry in the Sweepstakes.
